What’s Included Initial Intake and Assessment Once you enroll in the Six-Month Custom Nutrition Program, you will be granted access to download all required intake documents and forms. Please complete each form carefully and return them as instructed. These forms allow me to fully understand your health history, lifestyle, goals, and needs so your plan can be personalized accurately. 60-Minute Initial Nutrition Assessment (Phone or Zoom) After all completed forms are received and reviewed, we will schedule your 60-minute initial nutrition assessment. During this session, we will go over your intake forms in detail, discuss your current lifestyle, health history, challenges, and goals, and outline the nutrition and wellness approach that will guide your six-month journey. Personalized Nutrition & Wellness Lifestyle Plan Following your initial assessment, you will receive your personalized nutrition and lifestyle plan by email within a few days. This plan is thoughtfully designed around your goals, schedule, preferences, and current needs, and may include nutrition guidance, lifestyle recommendations, and supplemental considerations when appropriate. Weekly Personalized Meal Plans (24 Weeks) You will receive a customized 7-day meal plan every week for 24 weeks, tailored to your progress and evolving needs. Each plan includes a printable grocery list, meal planning tools, recipes, and simple meal prep tips to support consistency, balance, and long-term success. Ongoing Support & Check-In Calls You will receive weekly email support throughout the full six-month program, including regular check-ins, guidance, and plan adjustments as needed. In addition, the program includes bi-weekly scheduled progress check-in calls to review results, address challenges, and refine your plan to ensure you stay aligned with your goals. Please note that for clients located outside the U.S. and Canada, all scheduled calls will be conducted via WhatsApp/Zoom, based on your preference.
